Orlistat – a drug, a specific inhibitor of gastrointestinal lipases, which carry out the digestion of fats.Best known under the trade name “Xenical”, Roche (the company) and “Orsoten”. Used, including, as a means to reduce weight.

Mechanism of action

The preparation has high lipophilicity and mixed with fat droplets. Orlistat is covalently bound to the active site of pancreatic and gastric lipases, thereby inactivating them. Because inhibition of gastrointestinal lipases kishna triglycerides can not penetrate into the blood. This creates an energy deficit, which leads to the mobilization of fat from the depot. Under the influence of Xenical reduces weight mainly vistsero-abdominal fat. As Orlistat reduces hypercholesterolemia – owing to reduce free fatty acids and monoglycerides in the intestinal lumen – reduced cholesterol solubility which reduces its ability to penetrate the blood. Due to the increased sensitivity of tissues normalizing the blood lipid composition of insulin, reduces hyperinsulinemia, which also helps to reduce weight vistsero-abdominal fat.

Research

According to the multi-center, placebo-controlled studies, after 52 weeks of use of Xenical in the standard-dose body weight decreased by 6.2% from the original, insulin levels decreased by 18%.

In 60% of obese patients orlistat reduced weight by more than 10%, significantly reduced waist circumference. A significant decrease in body weight was achieved in patients with a normal state of carbohydrate metabolism, and in patients with type 2 diabetes. Orlistat has confirmed its high efficacy and safety in a large number of randomized placebo-controlled trials, the longest of which was to investigate XENDOS (continuous treatment ksenikalom 4 years).

During the third phase of clinical tests orlistat it found that it reduces the level of triglycerides, total cholesterol, LDL, glycated hemoglobin, blood pressure decreases.
